VitalHealth Software (VHS) was founded as collaboration between the Noaber Foundation (NL) and Mayo Clinic (US) with the objective of delivering an economic and social return. The Mayo Clinic is one of the world's largest integrated providers of healthcare. The Noaber Foundation is a Dutch social venturing firm in the healthcare information technology sector. VitalHealth Software delivers web-based software solutions for personalized health management. VitalHealth Software has been in business since 2006 and currently has over 110 people working from offices in the Netherlands, Germany, India and the United States. With over 65 people working in Resources and Development, VitalHealth Software has an extensive emphasis on this. VitalHealth Software's strategy is to extend its footprint in the EU market.

VitalHealth SoftwareVision - Personalized Health Management: Healthcare should be based on the individual needs of a patient, healthy or not, young or old, wealthy or poor, with or without chronic conditions, with or without the capability to manage their own health: Truly Personalized health Management. VitalHealth Software's mission is to deliver flexible software solutions that enable optimal treatment of chronic diseases, based on accepted protocols and guidelines, enhancing current healthcare IT solutions with maximum support or disease management in integrated care networks. Core target sectors are: Primary care networks, Hospitals, Mental Care Providers and Rehabilitation.

Software Platform: VitalHealth Software is the first global software vendor offering a web-based platform for chronic disease management that is flexible enough to enable the creation of integrated health management solutions tailored to both medical professionals and patients. This platform has been developed based on the following characteristics: Modelling-based software development; supports for roles within the care network; process-orientated care rule-based decision support and open platform architecture. The VitalHealth Platform is used by a growing network of partners to create 4th and 5th generation electronic health records.
